Position Overview

Location: Community-based role with flexible working options.

Contract: Temporary position covering maternity leave.


The purpose of this role is to deliver tailored support services to individuals experiencing dementia and related memory challenges. The aim is to assess their requirements and connect them with appropriate resources, enhancing their overall quality of life and empowering them to manage their circumstances effectively. Collaboration with various professionals, including healthcare providers and social service agencies, may be necessary to ensure comprehensive support.


Key Responsibilities:

  • Assist individuals impacted by dementia, along with their families and caregivers, in accessing essential services.
  • Enhance the well-being of clients, promoting autonomy and control over their lives.
  • Identify client needs and facilitate access to relevant information and services.
  • Advocate for our services and cultivate relationships with health and social care professionals.

Candidate Profile:

This role is ideal for a motivated and organized individual with a passion for supporting those affected by memory disorders and dementia. Candidates should demonstrate their qualifications and experience in their application. A strong understanding of client needs and confidentiality is crucial for providing effective support.


Successful candidates will possess:

  • Exceptional communication and organizational skills to manage client caseloads efficiently.
  • Adeptness in building professional relationships and networking within the community.
  • Empathy and a non-judgmental attitude, with a commitment to inclusivity.
  • Proficiency in IT tools, including Microsoft Office and Salesforce.
  • Effective time management skills, with the ability to work both independently and collaboratively.
  • Capability to travel independently within the designated area.
  • Experience in generating localized reports.
