Crisis Resolution Specialist

We are seeking a highly skilled and experienced Crisis Resolution Home Treatment Team Practitioner to join our team at Powys Teaching Health Board. As a key member of our Crisis Resolution Home Treatment Service, you will play a vital role in providing intensive home-based support to individuals experiencing mental health crises.
Key Responsibilities:
- Provide high-quality, person-centered care to individuals in crisis, working collaboratively with multidisciplinary teams to develop and implement effective care plans.
- Conduct comprehensive assessments and risk assessments to identify individual needs and develop tailored support plans.
- Deliver intensive home-based interventions, including crisis management, risk assessment, and support planning.
- Work closely with service users, families, and carers to ensure seamless transitions and continuity of care.
- Participate in regular team meetings and contribute to the development of service improvement initiatives.
Requirements:
- Professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy.
- Experience in community mental health settings, with a focus on crisis resolution and home treatment.
- Excellent assessment, risk assessment, and planning skills, with the ability to work effectively in high-pressure situations.
- Strong communication and interpersonal skills, with the ability to build rapport with service users, families, and colleagues.
- Ability to work flexibly, including evenings, weekends, and bank holidays.
