Key Responsibilities- Provide high-quality, person-centered care to service users in their own homes, promoting independence and well-being.
- Work collaboratively with multidisciplinary teams to assess, plan, and deliver effective interventions, ensuring seamless transitions between services.
- Develop and maintain strong relationships with service users, their families, and carers, fostering a culture of trust and respect.
- Contribute to the development and implementation of individualized care plans, ensuring that service users' needs are met and their goals are achieved.
- Participate in regular team meetings, sharing knowledge, expertise, and best practices to enhance the quality of care.
- Professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy.
- Experience in community mental health settings, with a strong understanding of crisis resolution principles and practices.
- Excellent assessment, risk assessment, and planning skills, with the ability to work effectively in high-pressure situations.
- Strong communication and interpersonal skills, with the ability to build rapport with service users, families, and colleagues.
- Ability to work flexibly, including evenings, weekends, and bank holidays, as part of a 24-hour shift pattern.
- Postgraduate qualifications in mental health, such as a Master's degree or diploma.
- Experience in working with diverse populations, including those from minority ethnic backgrounds.
- Knowledge of current mental health legislation, policies, and guidelines.
- Experience in supervising and mentoring junior staff members.
