Key Responsibilities

  • Work collaboratively with service users and their families to provide intensive home treatment and support
  • Develop and implement individualized care plans to address the needs of service users
  • Provide education and training to service users and their families on mental health issues and crisis management
  • Collaborate with other healthcare professionals to ensure seamless care and support

Requirements

  • Professional qualification in mental health nursing, social work, or occupational therapy
  • Experience in community mental health services
  • Strong assessment and risk management skills
  • Ability to work flexibly within a 24-hour shift pattern
