About the Role

We are seeking a highly skilled and experienced Crisis Resolution Home Treatment Team Practitioner to join our team at Powys Teaching Health Board. As a key member of our Crisis Resolution Home Treatment Service, you will play a vital role in providing intensive home-based support to individuals experiencing mental health crises.

Key Responsibilities:

  • Provide comprehensive assessments and risk assessments to individuals in crisis
  • Develop and implement personalized care plans to support individuals in their recovery
  • Work collaboratively with multidisciplinary teams to ensure seamless care coordination
  • Provide education and support to individuals and their families to promote mental health and well-being
  • Contribute to the development and implementation of service policies and procedures

Requirements:

  • Professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy
  • Experience in community mental health settings, with a focus on crisis resolution and home treatment
  • Excellent assessment and risk assessment skills, with the ability to work in a fast-paced environment
  • Strong communication and interpersonal skills, with the ability to work effectively with individuals, families, and multidisciplinary teams
  • Ability to work flexibly, including evenings, weekends, and bank holidays
