We are seeking a highly skilled and experienced Crisis Resolution Home Treatment Team Practitioner to join our team at Powys Teaching Health Board. As a key member of our Crisis Resolution Home Treatment Service, you will play a vital role in providing intensive home-based support to individuals experiencing mental health crises.
Key Responsibilities- Provide high-quality, person-centered care to individuals in crisis, working collaboratively with them and their families to develop and implement effective care plans.
- Conduct comprehensive assessments and risk assessments to identify the individual's needs and develop a tailored support plan.
- Work closely with other healthcare professionals, including GPs, community mental health teams, and hospital staff, to ensure seamless care coordination and effective discharge planning.
- Develop and maintain strong relationships with service users, their families, and carers, providing emotional support and guidance throughout the crisis.
- Participate in regular team meetings and contribute to the development of service policies and procedures.
- Professional qualification in mental health nursing, social work, or occupational therapy.
- Experience in community mental health settings, with a focus on crisis intervention and home-based support.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals from diverse backgrounds.
- Ability to work flexibly, including evenings, weekends, and bank holidays, to meet the needs of service users.
- Strong assessment and risk management skills, with the ability to identify and mitigate potential risks.
