Crisis Resolution Home Treatment Team Practitioner

We are seeking a highly skilled and experienced Mental Health Specialist to join our Crisis Resolution Home Treatment Team as a Home Treatment Team Practitioner. The successful candidate will be responsible for providing intensive home-based treatment to individuals experiencing a mental health crisis, working closely with the service user and their family to develop and implement a personalized care plan.
Key Responsibilities- Conduct comprehensive assessments of service users to identify their needs and develop a tailored care plan.
- Provide intensive home-based treatment to individuals experiencing a mental health crisis, including medication management and therapy.
- Work collaboratively with the multidisciplinary team to ensure seamless care coordination and communication.
- Develop and maintain strong relationships with service users, their families, and other stakeholders to ensure effective care delivery.
- Participate in regular team meetings and contribute to the development of the team's goals and objectives.
- Professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy.
- Experience in community mental health settings, with a focus on crisis resolution and home treatment.
- Strong assessment and risk management skills, with the ability to work with complex and high-risk cases.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to work flexibly, including evenings, weekends, and on-call shifts.
