We are seeking a highly skilled and experienced Crisis Resolution Home Treatment Team Practitioner to join our team at Powys Teaching Health Board. As a key member of our Crisis Resolution Home Treatment Service, you will play a vital role in providing intensive home-based support to individuals experiencing mental health crises.

Key Responsibilities:

  • Provide high-quality, person-centered care to individuals in crisis, working collaboratively with them and their families to develop and implement effective care plans.
  • Conduct comprehensive assessments and risk assessments to identify the individual's needs and develop strategies to mitigate risks.
  • Work closely with other healthcare professionals, including psychiatrists, psychologists, and social workers, to ensure seamless care coordination and effective communication.
  • Develop and maintain strong relationships with community-based services, including housing, social services, and voluntary organizations, to ensure a comprehensive support network for individuals in crisis.
  • Participate in regular team meetings and contribute to the development of service policies and procedures.

Requirements:

  • Professional qualification in mental health nursing, social work, or occupational therapy.
  • Experience in community mental health settings, with a focus on crisis intervention and home-based support.
  • Excellent assessment and risk assessment skills, with the ability to develop and implement effective care plans.
  • Strong communication and interpersonal skills, with the ability to work effectively with individuals, families, and healthcare professionals.
  • Ability to work flexibly, including evenings, weekends, and bank holidays, to meet the needs of individuals in crisis.
