Position Overview

The Action Group is dedicated to providing Housing Support and Care at Home services for both Adults and Children. We collaborate closely with local authorities to deliver high-quality care.

Role: Support Worker in Housing & Community Support Services (HACSS)

Location: Assigned regional base as determined by the line manager

Supervision: Managed by the Practice & Wellbeing Leader

Compensation:

Unqualified: £24,402.92 - £24,890.98 per annum pro rata

Qualified: £24,890.98 - £25,338.37 per annum pro rata

Casual rate: £12 per hour

Sleepover rate (if applicable): £96 per night

Working Hours: Part-time, up to 10 hours per week (Monday to Thursday) from 7am to 9am

Eligibility: Must be a driver with access to a vehicle, *Female only.* This position is exempt under The Equality Act 2010 Schedule 9, Part 1. Male applicants are encouraged to apply for other available roles.

Core Responsibilities:

The primary objective of this role is to deliver compassionate, efficient, and effective support services around the clock to individuals utilizing The Action Group's offerings. This includes aiding individuals in skill development through supervision and participation in various support activities that enhance their opportunities and promote autonomy. The key responsibilities are outlined below:

  • Implement the TAG Key Worker system, which involves organizing support plans and case reviews with all relevant stakeholders, ensuring effective staff handovers, and maintaining accurate records.
  • Foster the physical and emotional wellbeing of those we support by assisting them in maintaining adequate self-care and conducting risk assessments.
  • Build meaningful and trusting relationships with individuals, helping them gain insights into their behaviors and reactions.
  • Encourage independence in daily living activities and provide support only when necessary.
  • Promote hobbies, interests, and community participation, including organizing outings and accompanying individuals to activities.
  • Assist with personal care as required.
  • Support individuals in enhancing their communication skills and promote self-advocacy.
  • Advocate for individuals when necessary, liaising with external organizations regarding benefits, education, and employment.
  • Engage with relatives and caregivers to ensure they play a significant role in the lives of those we support.
  • Research local services and amenities available to the individuals we assist.
  • Adapt to the varying needs of individuals, participating in flexible shift patterns, including evenings and weekends.
  • Occasionally cover for short-term absences of other Support Workers.

General Responsibilities:

  • Manage work time effectively and maintain organized filing systems.
  • Communicate promptly and clearly with colleagues and individuals we support.
  • Stay updated and comply with all TAG policies and procedures.
  • Uphold individual rights and responsibilities at all times.
  • Maintain confidentiality as required.
  • Assist colleagues and managers as needed.
  • Engage in Continuous Professional Development (CPD) and contribute to maintaining the Investors In People standard.
  • Ensure a healthy work-life balance.
