Role: Care Support Specialist
Service: Housing & Community Support Services (HACSS)
Location: Regional base as determined by management
Reporting To: Team Manager
Compensation:
Unqualified: £24,402.92 - £24,890.98 per annum pro rata
Qualified: £24,890.98 - £25,338.37 per annum pro rata
Casual rate: £12 per hour
Sleepover rate (if applicable): £96 per night.
Work Schedule: Part-time; 5 to 16 hours per week
Primary Objective:
The primary objective of this role is to deliver a compassionate, efficient, and effective support service to individuals utilizing The Action Group's offerings. This includes aiding individuals in skill development through supervision and participation in various support activities, thereby enhancing their opportunities and promoting personal choice.
Key Responsibilities
- Implement the TAG Key Worker system, including organizing support plans and case reviews in collaboration with relevant stakeholders, ensuring effective staff handovers, and maintaining comprehensive records.
- Support the physical and emotional well-being of individuals at all times, including assisting with self-care and conducting risk assessments.
- Foster meaningful and trusting relationships with individuals, helping them gain insights into their behaviors and reactions.
- Encourage independence in daily living activities, providing support only when necessary.
- Promote individuals' hobbies and interests, facilitating participation in community life, including organizing outings and accompanying individuals to activities.
- Assist with personal care as required.
- Work collaboratively with individuals to enhance their communication skills and promote self-advocacy.
- Advocate for individuals as needed, liaising with external organizations regarding benefits, education, and employment.
- Engage with relatives and caregivers, ensuring they play a significant role in the lives of those we support.
- Research local services and amenities available to individuals.
- Participate flexibly in various shift patterns to meet the needs of individuals, including evening and weekend work.
- Occasionally cover for Support Workers in other teams during short-term absences.
General Responsibilities
- Adhere to TAG's Mission, Aims & Objectives, and current Business Plan.
- Manage work time effectively and maintain organized filing systems.
- Communicate promptly and clearly with staff and individuals.
- Stay updated and comply with all relevant TAG policies and procedures.
- Respect individual rights and maintain confidentiality.
- Contribute to Continuous Professional Development (CPD) and uphold the Investors In People standard.
- Maintain a healthy work-life balance.
