Job Summary
We are seeking an experienced Field Care Supervisor to join our team, responsible for supporting the delivery of exceptional care services in the community.
Key Responsibilities
- Support the planning and coordination of care worker rotas, ensuring effective coverage and high-quality care delivery.
- Conduct regular reviews of individual services, including risk assessments and needs evaluations.
- Participate in multi-disciplinary reviews of service users' needs, ensuring comprehensive and holistic care planning.
- Provide on-site supervision and support to care workers, promoting best practice and quality care delivery.
- Collaborate with care coordinators and other stakeholders to ensure seamless care delivery and effective communication.
Requirements
- Experience in carrying out initial risk and needs assessments, with a strong understanding of care planning and coordination.
- Ability to work in partnership with service users, families, carers, and healthcare professionals to deliver high-quality care.
- Strong communication and interpersonal skills, with the ability to build effective relationships with stakeholders.
- Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
