Job Summary
We are seeking an experienced Field Care Supervisor to join our team. As a Field Care Supervisor, you will play a vital role in supporting the delivery of our care services, ensuring that our clients receive the highest level of care and support.
Key Responsibilities
- Support the planning and delivery of care services, working closely with Care Coordinators to ensure that care plans are tailored to meet the individual needs of our clients.
- Conduct regular reviews of individual services, including risk assessments and needs assessments, to ensure that our clients receive the best possible care.
- Supervise and support care workers in the field, providing guidance and training to ensure that they are equipped to deliver high-quality care.
- Participate in multi-disciplinary reviews of service users' needs, working with healthcare professionals and other stakeholders to ensure that our clients receive comprehensive support.
- Shadow new care workers in the field to ensure their competence and provide on-site observation and evaluation of care worker practice.
Requirements
- Experience in carrying out initial risk and needs assessments in respect of individual service users.
- Ability to work in partnership with service users, their family, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of holistic packages of individual care support.
- Strong communication and interpersonal skills, with the ability to build strong relationships with colleagues, clients, and other stakeholders.
- Ability to work independently and as part of a team, with a flexible approach to working hours and a willingness to work on-call.
