Position Overview

The Action Group is dedicated to providing exceptional Housing Support and Care at Home services for both Adults and Children. Our mission is to enhance the quality of life for those we serve through compassionate and effective support.

Role: Care Support Specialist
Service: Housing & Community Support Services (HACSS)

Location: As designated by the line manager

Reporting To: Team Manager

Compensation:

Unqualified: £24,402.92 - £24,890.98 per annum pro rata
Qualified: £24,890.98 - £25,338.37 per annum pro rata
Casual Rate: £12 per hour
Sleepover Rate (if applicable): £96 per night

Work Schedule: Part-time; 5 to 16 hours per week

Primary Objective:

The primary objective of this role is to deliver a compassionate, efficient, and effective 24/7 support service to individuals utilizing The Action Group's services. This includes assisting individuals in skill development through supervision and participation in various support activities, enriching their opportunities and promoting choice.

Key Responsibilities

  • Implement the TAG Key Worker system, organizing support plans and case reviews in collaboration with relevant stakeholders.
  • Promote the physical and emotional wellbeing of individuals, ensuring adequate levels of self-care and conducting risk assessments.
  • Foster meaningful and trusting relationships with individuals, aiding them in gaining insight into their behaviors and reactions.
  • Encourage independence in daily living activities, providing support only when necessary.
  • Facilitate participation in hobbies, interests, and community activities, including organizing outings and accompanying individuals to events.
  • Assist with personal care as required.
  • Support individuals in enhancing their communication skills and promote self-advocacy.
  • Advocate for individuals when necessary, liaising with external organizations regarding benefits, education, and employment.
  • Engage with relatives and carers, ensuring their involvement in the individual's life and addressing any concerns.
  • Research local services and amenities available to individuals.
  • Participate flexibly in various shift patterns, including evenings and weekends as needed.
  • Occasionally cover for short-term absences of Support Workers in other teams.

General Responsibilities

  • Adhere to TAG's Mission, Aims & Objectives, and current Business Plan.
  • Effectively manage personal work time and maintain organized filing systems.
  • Communicate promptly and clearly with staff and individuals.
  • Stay updated and comply with all TAG policies and procedures.
  • Respect individual rights and maintain confidentiality.
  • Contribute to Continuous Professional Development (CPD) and uphold the Investors In People standard.
  • Maintain a healthy work-life balance.
