The Action Group provides essential Housing Support and Care at Home services tailored for both Adults and Children, collaborating closely with local authorities.

Position: Care Support Specialist

Service: Housing & Community Support Services

Location: As designated by the line manager

Supervision: Team Manager

Compensation:

Unqualified: £24,402.92 - £24,890.98 per annum pro rata

Qualified: £24,890.98 - £25,338.37 per annum pro rata

Casual rate: £12 per hour

Sleepover rate (if applicable): £96 per night.

Working Hours: Part-time; ranging from 5 to 16 hours

Core Responsibilities:

The primary objective of this role is to deliver compassionate, efficient, and effective support services to individuals utilizing The Action Group's offerings. This encompasses aiding individuals in skill development through supervision and engagement in various support activities, thereby enhancing their opportunities and fostering choice.

Key Responsibilities

  • Implement the TAG Key Worker framework, which includes organizing support plans and case reviews in collaboration with relevant stakeholders, ensuring seamless staff handovers, and maintaining comprehensive records.
  • Promote the physical and emotional wellbeing of clients, including assisting with self-care and conducting risk assessments.
  • Foster meaningful and trusting relationships with clients, aiding them in understanding their behaviors and reactions.
  • Encourage independence in daily living tasks while providing support as necessary (e.g., household chores, meal preparation, and administrative tasks).
  • Facilitate engagement in hobbies, interests, and community activities, including organizing outings and accompanying clients to events.
  • Provide personal care assistance as required.
  • Support clients in enhancing their communication skills and promote self-advocacy.
  • Advocate for clients when necessary, liaising with external organizations regarding benefits, education, and employment.
  • Collaborate with family members and caregivers, ensuring their involvement in the client's life.
  • Research local services and amenities available to clients.
  • Participate in various shift patterns as dictated by client needs, including evenings and weekends.
  • Occasionally cover for absent Support Workers in other teams.

General Responsibilities

  • Adhere to TAG's Mission, Aims & Objectives, and current Business Plan.
  • Effectively manage personal work time.
  • Maintain organized paperwork and filing systems relevant to the role.
  • Engage with all individuals with respect and consideration.
  • Ensure clear and prompt communication with staff and clients.
  • Stay informed and comply with all TAG policies and procedures.
  • Uphold individual rights and responsibilities at all times.
  • Maintain confidentiality as required by the role.
  • Assist colleagues and management as needed.
  • Take responsibility for personal Continuous Professional Development (CPD).
  • Manage work-life balance effectively.
