  • Permanent roles are available within the Initial Response Team in Intake & Assessment.
  • Social Workers may participate in care and private law processes.

Candidate Profile

  • The ideal applicant should have extensive experience in assessment, care planning, person-centered approaches, and care management.
  • Collaborative engagement with partners to identify and meet individuals' care and support needs.
  • Registered Social Worker with robust assessment capabilities, skilled in producing clear documentation, and excelling in teamwork, communication, organization, and IT skills.
