Community Mental Health Nurse

Job Summary:
We are seeking a skilled and motivated Mental Health Nurse to join our team in Central Aberdeenshire. As a Community Mental Health Nurse, you will work with individuals who have been referred from GP Practices, providing care and support to help them manage their mental health.
Key Responsibilities:
- Assess and manage a caseload of patients under the supervision of the Team Leader
- Work collaboratively with other healthcare professionals to provide comprehensive care
- Develop and implement individualized care plans to meet the needs of patients
- Provide education and support to patients and their families
- Contribute to the development of the team and service
Requirements:
- Registered Mental Health Nurse qualification
- Experience working with older adults
- Ability to work independently and as part of a team
- Good communication and assessment skills
- Car driver/owner
