Job Summary

We are seeking a highly motivated and enthusiastic Health Care Support Worker to join our Kirklees Intensive Support Team (IST). The successful candidate will work closely with a small number of service users, aiming to reduce hospital admissions and placement breakdowns.

Main Duties
  • Provide high-quality care and support to individuals with learning disabilities and/or Autistic Spectrum Disorder.
  • Work collaboratively with the IST multidisciplinary team to provide an intensive, wrap-around service.
  • Assess and develop Positive Behavioral Support plans and interventions.
  • Liaise with families and support staff to model and coach interventions.
  • Develop and maintain meaningful therapeutic relationships with service users.
Requirements
  • GCSE A-D or grade 3-9 in Maths and English or equivalent.
  • Apprenticeship level 3 or equivalent in Health & Social Care.
  • Experience in providing personal care and related procedures for those with health/social needs.
  • Ability to work effectively, professionally, and independently or as part of a multidisciplinary team.
