Job Summary

We are seeking a highly motivated and enthusiastic Health Care Support Worker to join our Intensive Support Team (IST) in Kirklees. The successful candidate will work closely with a small number of service users, providing intensive assessment, formulation, and intervention within a Positive Behavioral Support Model.

Main Duties
  • Provide high-quality care and support to service users, promoting their recovery and rehabilitation potential.
  • Develop and maintain meaningful therapeutic relationships with service users, adopting a polite, positive, and empathetic approach.
  • Assess situations, identify potential problems, and take appropriate action to ensure service user safety.
  • Contribute to the care planning process, understanding the principles underlying care planning and implementing care within established policies and procedures.
  • Work effectively as part of a multidisciplinary team, demonstrating initiative, decision-making, and prioritization skills.
Requirements
  • GCSE A-D or grade 3-9 in Maths and English or equivalent.
  • Apprenticeship level 3 or equivalent, such as RQF NVQ level 3 in Health & Social Care or equivalent.
  • Care Certificate or required to achieve within 12 weeks of commencement.
  • Ability to work flexibly on a 24hr shift pattern to meet the needs of service users.
  • Excellent communication and interpersonal skills.
  • Good IT/keyboard skills.
  • Diversity/Cultural understanding.
  • Knowledge of service user safety, including prevention and management of violence and aggression, moving and handling, etc.
