Job Summary

We are seeking a highly motivated and enthusiastic Health Care Support Worker to join our Kirklees Intensive Support Team (IST). The successful applicant will work in a proactive and intensive environment, providing support to individuals with complex needs and challenging behaviors.

Main Duties
  • Provide high-quality care and support to individuals with learning disabilities and/or Autistic Spectrum Disorder.
  • Work collaboratively with the IST multidisciplinary team to develop and implement Positive Behavioral Support plans.
  • Assess and address the needs of service users, including their physical, emotional, and social needs.
  • Develop and maintain meaningful relationships with service users, families, and carers.
  • Contribute to the care planning process and implement care within established policies and procedures.
Requirements
  • GCSE A-D or grade 3-9 in Maths and English or equivalent.
  • Apprenticeship level 3 or equivalent in Health & Social Care or equivalent.
  • Care Certificate or required to achieve within 12 weeks of commencement.
  • Ability to work flexibly on a 24hr shift pattern to meet the needs of service users.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
