We are seeking a highly motivated and enthusiastic Health Care Support Worker to join our Kirklees Intensive Support Team (IST). The successful candidate will work closely with a small number of service users, aiming to reduce hospital admissions and placement breakdowns.
Main Duties- Provide high-quality care and support to individuals with learning disabilities and/or Autistic Spectrum Disorder.
- Work creatively with service users in various settings across the community.
- Assess and develop Positive Behavioral Support plans and interventions.
- Liaise with families and support staff to offer modelling and coaching of interventions.
- Develop and maintain meaningful therapeutic relationships with service users.
- GCSE A-D or grade 3-9 in Maths and English or equivalent.
- Apprenticeship level 3 or equivalent in Health & Social Care.
- Experience in providing personal care and related procedures for those with health/social needs.
- Ability to work effectively, professionally, and independently or as part of a multidisciplinary team.
