About the Role
We are seeking a highly skilled and experienced Geriatrician to join our team at Southern Health NHS Foundation Trust. As a Consultant Geriatrician, you will play a key role in providing high-quality medical care to older patients who require specialized attention.
Main Responsibilities
- Provide expert clinical opinion and take full responsibility for the care of patients with complex geriatric conditions.
- Work closely with colleagues in primary, secondary, and intermediate care to develop and implement proactive community services for older people living with frailty.
- Contribute to the development and implementation of admission prevention strategies, virtual wards, and advance care planning initiatives.
- Collaborate with multidisciplinary teams to deliver high-quality care and improve patient outcomes.
