About the Role
We are seeking a highly skilled and experienced Geriatrician to join our team at Southern Health NHS Foundation Trust. As a Consultant Geriatrician, you will play a key role in providing high-quality medical care to older patients, working closely with colleagues in primary, secondary, and intermediate care.
Main Responsibilities
Key responsibilities of the role include:
- Providing expert clinical opinion across a wide range of problems within General Internal Medicine and Geriatric medicine
- Taking full and independent responsibility for the care of patients
- Working closely with colleagues to develop and implement proactive community services for older people living with frailty
- Contributing to admission prevention, virtual wards, and end-of-life care initiatives
