Position Overview

We are pleased to announce an opportunity for an Advanced Clinical Pharmacist to become part of a dedicated team providing specialized clinical pharmacy and medication management services across various healthcare settings, including community hospitals and mental health facilities. This role involves participation in essential clinical discussions and multidisciplinary team meetings.

Key Responsibilities

The successful candidate will be responsible for:

  • Assisting the Senior Clinical Pharmacist in evaluating and addressing medication-related incidents, while also contributing to the analysis of trends for clinical teams.
  • Ensuring that all relevant Trust policies, guidelines, and standard operating procedures are current, safe, and applicable.
  • Promoting adherence to best practices in medication prescribing and administration, while facilitating multidisciplinary training on medication management.
  • Implementing strategies for optimal medication use to enhance patient outcomes in community hospitals and mental health settings.
  • Ensuring compliance with legal frameworks and best practices regarding the safe handling of medications, including controlled substances.
  • Providing expert advice on the safe and economical use of medications, while monitoring pharmaceutical expenditures and reporting findings to relevant stakeholders.

Communication and Collaboration

Effective communication is essential in this role. The Advanced Clinical Pharmacist will:

  • Facilitate smooth transitions of care for medications by fostering positive relationships with colleagues.
  • Act as an authorized witness for the disposal of controlled substances in accordance with applicable regulations.
  • Represent the pharmacy team in relevant committees and groups as necessary.
  • Deliver coordinated medication information to patients during their hospital stay and as part of discharge planning.

Administrative Duties

In addition to clinical responsibilities, the role includes:

  • Maintaining accurate and comprehensive patient records reflecting the pharmacist's activities.
  • Participating in clinical audits to ensure adherence to guidelines and to support service development.
  • Documenting all clinical pharmacy interactions and interventions in the Trust's electronic system.

Qualifications and Experience

To qualify for this position, candidates must possess:

  • A Master's degree in pharmacy or an equivalent qualification.
  • Registration with the General Pharmaceutical Council (GPhC).
  • Post-registration experience in a hospital or community pharmacy setting.
