Job Overview

This position is a developmental role, where if appointed at Band 6, you will be assigned a set of competencies to complete, which will be evaluated during your appraisal. Upon successful completion, you will advance to Band 7.

We are seeking a dedicated pharmacist to join our team, contributing to our specialized clinical pharmacy and medication optimization services. The successful candidate will engage across three pharmacy disciplines, collaborating with other pharmacy professionals to provide exceptional service throughout the organization.

The role will involve working across multiple locations, with responsibilities shared between the hospital pharmacy department and associated mental health wards, as well as community rehabilitation facilities.

Our service is transitioning towards electronic prescribing and medication administration, and the successful candidate will play a pivotal role in the early stages of this development, helping to shape the system and advocate for its benefits in patient care and safety.

Key Responsibilities

We are looking for an individual who can positively impact the clinical management of medications for those receiving care from our services. The successful candidate will collaborate closely with multidisciplinary teams to ensure the safe and effective use of medications within the Trust.

The successful candidate will also contribute to the clinical pharmacy services provided to all inpatient units and the dispensary, delivering tailored, patient-centered pharmacy care to those in need across the service.

The role requires the development and demonstration of a range of pharmacy-related skills, including addressing medication information inquiries, supporting procurement and formulary decisions, managing clinical trial medications, and training other staff members both within and outside the pharmacy team.

Job Duties

  • Assist with the medication management service across various healthcare facilities.
  • Enhance patient care in designated wards/teams by engaging in medication management initiatives alongside pharmacy technicians, including patients' own drugs, self-administration, medication reconciliation, and supply management.
  • Provide counseling and education to service users regarding their medications, offering information and advice to service users, caregivers, and other healthcare professionals.
  • Actively participate in multidisciplinary team meetings or case conferences to identify medication-related issues, resolve transfer of care challenges, and provide expert advice.
  • Support and review prescribing practices, monitoring medication use and treatment outcomes.
  • Facilitate the education and training of medication-related topics for Trust staff, external stakeholders, and service users.
  • Engage in additional pharmacy activities such as handling clinical trial medications, conducting audits, and performing near-patient testing of blood samples as part of medication monitoring.

Required Knowledge & Skills

Essential

  • Proficiency in medication management skills, including patient interviews, medication reconciliation, and discharge counseling.

Desirable

  • Understanding of key aspects of medication use in palliative and mental health contexts.

Experience

Essential

  • Prior experience in a hospital setting.
  • Experience working within a healthcare team to achieve positive patient outcomes.

Desirable

  • Previous experience in a hospital pharmacy environment.
  • Experience supporting mental health services within a pharmacy context.
