Job Summary

Lakeside Healthcare Group is seeking a highly skilled Clinical Pharmacist to join our team as a Medicines Optimisation Specialist. This is an exciting opportunity for a motivated and experienced pharmacist to work in a dynamic and forward-thinking organisation.

Key Responsibilities
  • Provide expert clinical pharmacy services to patients in general practice, focusing on medicines optimisation and rational prescribing.
  • Work collaboratively with GPs, nurses, and other healthcare professionals to improve patient outcomes and reduce healthcare costs.
  • Conduct face-to-face medication reviews, manage long-term conditions, and ensure safe prescribing practices.
  • Develop and implement quality improvement initiatives to enhance medicines management and patient care.
  • Contribute to the development and implementation of new services and treatments that have medicinal components.
  • Monitor and audit prescribing practices to ensure compliance with local and national guidelines.
  • Provide training and education to practice staff on medicines management and optimisation.
Requirements
  • Registration as a Pharmacist with the General Pharmaceutical Council.
  • Evidence of Continuing Professional Development (CPD).
  • Masters degree in pharmacy (MPharm) or equivalent.
  • Independent prescriber with experience of managing chronic disease.
  • Postgraduate diploma in Clinical Pharmacy or completion of the CPPE Pathway.
  • Minimum of 2 years post-qualification experience and 1-2 years experience in General Practice.
Desirable Qualifications and Experience
  • Membership of the Royal Pharmaceutical Society.
  • Experience of working in multiple sectors of pharmacy (Community Pharmacy, Hospital Pharmacy, CCGs, Industry).
  • Experience of using SystmOne.
  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
