Job Overview

We are seeking a dedicated and motivated Clinical Pharmacy Specialist to become a vital member of our collaborative pharmacy team.

Our team is committed to providing comprehensive pharmaceutical services to mental health and community health sectors, focusing on adults and the elderly.

While experience in mental health is preferred, we welcome applicants who are eager to transition into this specialty and will provide the necessary support for that journey.

The successful candidate will be required to participate in a Trust-wide 'out-of-hours' service, collaborating with fellow mental health pharmacists. This role necessitates the ability to travel to the workplace during non-standard hours.

Key Responsibilities

We are looking for an individual who can make a positive impact on the clinical management of patients facing mental health challenges. The chosen candidate will work alongside multidisciplinary teams to ensure the safe and effective use of medications and will support numerous outpatients through various services.

The role includes contributing to the clinical pharmacy services for all inpatient units and the dispensary. Responsibilities will also encompass addressing medication information inquiries, engaging in clinical trials, and developing policies and procedures. We expect the individual to deliver tailored, patient-centered pharmaceutical care to all who require it across the service.

Responsibilities Overview

- Assist with the medicines management and dispensing services.

- Support the medicines management service across various healthcare facilities.

- Enhance patient care in designated wards/teams by:

  a. Participating in medication reconciliation and interviews with service users.

  b. Engaging in medicines management initiatives, including patients' own drugs, self-administration, and medicines supply.

  c. Providing counseling and education to service users regarding their medications, along with offering information and advice to service users, caregivers, and other healthcare professionals.

  d. Actively participating in discharge planning to ensure a smooth transition of medication management into primary care.

  e. Contributing to multidisciplinary team meetings to identify medication-related issues and provide expert advice.

  f. Reviewing and monitoring prescription records and treatment outcomes.

  g. Supporting evidence-based prescribing in accordance with local and national guidelines.

- Facilitate the education and training of pharmacy-related matters for staff and service users.

Qualifications & Experience

Essential

  • Degree in Pharmacy from a recognized institution.
  • Registered practicing pharmacist with the General Pharmaceutical Council (GPhC).

Desirable

  • Postgraduate qualification in clinical pharmacy.
  • Postgraduate certificate in Psychiatric Therapeutics.

Experience Requirements

  • Minimum of 2 years' post-registration experience as a pharmacist.

Desirable

  • Experience in hospital pharmacy settings.
  • Experience in mental health pharmacy.
