We are seeking a motivated pharmacist to join our team, contributing to our specialized clinical pharmacy and medication optimization services. The successful candidate will collaborate across three pharmacy disciplines with fellow pharmacy professionals to provide exceptional service throughout the organization.

The role will involve working across three locations, dividing time between the pharmacy department and associated mental health wards, as well as community rehabilitation wards.

Our service is currently transitioning to electronic prescribing and medication administration, allowing the successful candidate to engage in the early stages of this initiative. You will play a crucial role in shaping the system and advocating for its benefits to enhance patient care and safety.

Key Responsibilities

We are looking for an individual who can make a positive impact on the clinical management of medications for those receiving care from our services. The successful candidate will collaborate closely with multidisciplinary teams to ensure the safe and effective use of medications within the Trust.

The chosen candidate will enhance the clinical pharmacy services provided to all inpatient units and the dispensary, delivering tailored, patient-centered pharmacy care to those in need across the service.

The role requires the development and demonstration of a range of pharmacy-related skills, including addressing medication information inquiries, supporting procurement and formulary decisions, managing clinical trial medications, and training other personnel both within and outside the pharmacy team.

Responsibilities Overview

  • Assist with the medication management service at designated healthcare facilities.
  • Enhance patient care in specified wards/teams alongside pharmacy staff by participating in medication management initiatives, including patient-owned medications, self-administration, medication reconciliation, and medication supply.
  • Provide counseling and education to service users regarding their medications and offer information and advice to service users, caregivers, and other healthcare professionals.
  • Actively participate in multidisciplinary team meetings or case conferences to identify medication-related issues, resolve transfer of care challenges, and provide expert advice.
  • Support and review prescribing practices and monitor medication use and treatment outcomes.
  • Facilitate the education and training of medication-related topics for staff, external stakeholders, and service users.
  • Engage in additional pharmacy activities such as managing clinical trial medications, conducting audits, and performing near-patient testing of blood samples as part of medication monitoring.

  • Proficiency in medication management skills (patient interviews, medication reconciliation, discharge counseling).

  • Understanding of key aspects of medication use in palliative and mental health contexts.

  • Prior experience in a hospital setting.
  • Experience working within a healthcare team to achieve positive patient outcomes.
