About the Role

We are seeking an experienced and skilled Senior Clinical Practitioner to join our team at West London NHS Trust. As a key member of our Specialist Older Adults Mental Health (SOAMHS) service, you will provide clinical leadership and expertise to support the delivery of high-quality care to older adults with mental health needs.

Key Responsibilities
  • Provide clinical leadership to the SOAMHS service, ensuring that patients receive assessment and therapeutic intervention to maximize their independence, safety, and wellbeing.
  • Deputize for the team manager with the day-to-day running of the service, including managing clinical risks and complex cases.
  • Develop and implement innovative practice to improve patient outcomes and service delivery.
  • Work closely with patients, carers, statutory and voluntary agencies, and other healthcare professionals to provide comprehensive care.
  • Hold a complex caseload of patients and provide individualized care planning and support.
Requirements
  • Registered Practitioner (nursing) appropriate to the job role.
  • Degree/diploma level qualification supplemented by specialist qualification and/or experience.
  • Extensive post-qualifying experience in relevant service area.
  • Experience of supervising staff and students.
  • Experience of managing clinical risks with complex cases.
