About the Organization

We are in search of a passionate and driven Physiotherapist to become part of our community rehabilitation team. You will collaborate with a friendly and skilled group of Physiotherapists, Occupational Therapists, and Community Assistant Practitioners.

This role provides the chance to work with patients in community inpatient facilities and residential care, as well as delivering rehabilitation services in their homes.

You will have opportunities to enhance your clinical expertise through our training programs and will engage in regular supervision. Additionally, there are chances to develop your leadership skills by overseeing junior staff.

Responsibilities Include:

  • Conducting thorough assessments and developing care plans for patients with chronic conditions, elderly individuals, those requiring palliative care, and rehabilitation needs to promote quality of life and independence.
  • Collaborating with the integrated team to facilitate timely discharges from healthcare facilities.
  • Working within the integrated team to avert unnecessary hospital admissions.
  • Coordinating with healthcare professionals and various agencies to ensure a seamless service for our clients.

Clinical Duties

The successful candidate will:

  • Perform comprehensive physiotherapy evaluations for patients, including those with complex conditions, utilizing investigative and clinical reasoning skills to inform diagnosis.
  • Develop and implement tailored physiotherapy management and treatment strategies in collaboration with the integrated team and the patient.
  • Recognize the need for Assistive Technology and make referrals as necessary.
  • Assess the holistic needs of patients, effectively communicating complex information regarding their assessment, diagnosis, prognosis, and treatment plans.
  • Adjust explanations to accommodate any barriers to understanding, ensuring informed consent and cooperation.
  • Utilize persuasive communication skills to encourage patient compliance with treatment plans.
  • Liaise with GPs, social services, inpatient teams, and other healthcare professionals to ensure comprehensive care coordination.
  • Participate in multidisciplinary meetings as required.
  • Manage and prioritize personal workload in alignment with service objectives.
  • Maintain accurate patient records and ensure timely data entry.
  • Organize specialized services within the integrated team.
  • Demonstrate manual dexterity and coordination for patient assessment and treatment.
  • Assist in the procurement of supplies and equipment.
  • Adhere to budgetary constraints.
  • Ensure informed consent is obtained prior to initiating any interventions.
  • Follow infection control protocols and utilize universal precautions.
  • Exhibit basic IT skills and proficiency in standard keyboard operations.

Qualifications and Experience

Essential Qualifications:

  • Diploma or first-level degree in Physiotherapy.
  • Current registration with the Health and Care Professions Council (HCPC).
  • Completion of a Clinical Educators Course.

  • Experience within the NHS.
  • Proven experience in a community-based role.
  • Experience working collaboratively within a multidisciplinary team.
