Position Overview

Compensation: £38,092 - £45,389 annually (including a £3,000 Market Supplement) plus an additional £2,000 upon completion of three months (for external candidates), and a further £2,000 after one year, continuing annually thereafter for both internal and external candidates.

As a vital member of our multidisciplinary Hospital Team, the Team Support Manager will collaborate with both health and social care professionals to provide outstanding social work services that significantly impact adults requiring social care. As a fully qualified Social Worker with expertise in safeguarding, intricate case management, resource distribution, and funding, you will be dedicated to empowering individuals to achieve optimal outcomes, ensuring their safety and independence, allowing them to lead fulfilling lives.

Key Responsibilities

In your role as Team Support Manager, you will oversee and guide our Senior Case Workers and Social Workers while managing a small yet complex caseload. You will share the responsibility of leadership within the broader team, supporting our duty function, stepping in for the Team Manager as needed, and contributing to the strategic planning and execution of services in alignment with our governance standards.

Your exceptional communication and relationship-building abilities will enable you to foster successful and constructive partnerships with individuals and professional health staff, both within and outside the hospital team.

Job Duties

Our teams operate across five major hospitals, each with a dedicated Social Work Discharge Team. We thrive in a dynamic, multidisciplinary environment, collaborating closely with colleagues in Health, Social Care, providers, and the voluntary sector to ensure safe, timely, and sustainable discharge plans. Our focus is on supporting recovery and rehabilitation, identifying long-term needs, safeguarding, and maximizing independence. We work across various settings, including acute and community hospitals, to plan ongoing care for individuals in their homes as well as short-term and long-term care services.

Your Role

As a Team Support Manager for Adults' Health and Care, you will deliver person-centered, safe, and secure services through the application of social care legislation and local government requirements, guided by professional training.

Your responsibilities will include assisting the manager with daily team operations, including case allocation, providing support and guidance to the team in managing complex cases, overseeing staffing levels and schedules, and offering regular supervision to staff. You will also represent the organization in multidisciplinary meetings, assist with safeguarding decisions, and manage triage and onward care functions.

Qualifications

Essential

  • Professional registration as a Qualified Social Worker with Social Work England (SWE).
  • Comprehensive understanding of applying a strengths-based approach to practice.
  • Knowledge of the philosophies, professional social work issues, and legal frameworks relevant to specific client groups and social services statutory duties.
  • Advanced knowledge of local authority statutory duties.
  • Awareness of equality and diversity issues in social work practice, emphasizing safety, dignity, and empowerment.
  • Understanding of data protection and confidentiality, with a commitment to accurate and timely record-keeping.
  • Experience in professional practice, including working with interpreters.

Skills

  • Ability to identify problems and support staff in using a person-centered approach to find effective solutions.
  • Capacity to work independently and collaboratively, fostering the same in staff.
  • Ability to recognize staff development needs and support colleagues in challenging situations.
