Key Duties/Responsibilities Clinical To undertake patient examination and clinical history, taking into account the holistic assessment of physical, psychological, emotional and social needs. To make sound, autonomous, informed clinical decisions utilising analytical and judgment skills to assess and advise within specialist field of expertise. Plans specialised programmes of care and provides highly specialised advice to the multidisciplinary team, patients, relatives, and carers. To establish, undertake and monitor nurse-led clinics, including the assessment, review, and allocation of patients to Clinical Haematology clinics / professionals within and external to the Clinical Haematology service ensuring that each patient is placed in the correct treatment pathway at the appropriate time and necessary investigations and treatments are organised.

To lead on the planning, development, and evaluation of clinical protocols/guidelines for specialist area. Works autonomously to manage their own caseload of patients whilst working as part of a multidisciplinary team. To lead the specialist team in ensuring compliance with local and national standards. Enables patient choice and involvement and initiates appropriate action.

Leads the coordination and development of patient care pathway and liaises across organisational boundaries. Provides clinical advice and support to healthcare professionals within and across organisational boundaries. Provides clinical advice in accordance with current evidence-based practice. Participates in case conferences in relation to future management or discharge arrangements.

Ensures patients and their families are given appropriate oral and written information regarding planned care. Maintains accurate patient records and ensures all relevant information is documented in the patients medical and nursing records. Identify measure and monitor key clinical outcomes in this area of practice, demonstrating continuous improvements. Work as an independent non-medical prescriber (adhering to Trust policy) with the ability to initiate and adjust medication within your field of expertise, including prescription of blood transfusions.

Leads the MDT with consultants and specialist registrars to discuss complex cases and future management. Education To lead the development, planning, and implementation of a range of specialist educational and training programmes for all healthcare professionals, patients and carers. Lead in the teaching and development of healthcare professionals, patients, and carers. To Co-ordinate and undertake clinical supervision and mentorship to junior and new nurse specialists / nurses / medical staff as required.

Identifies, assists, and meets the educational needs of the patient and their family. Maintains and updates evidence-based knowledge and skills. Fulfils all mandatory training requirements for the Trust and assists others within the department to do so. Undertakes health promotion across organisational boundaries.

Management Manages own caseload, and coordinates patient journey across business groups and organisational boundaries. Works in close collaboration with other health care professionals across organisational boundaries to ensure service meets patients needs. Acts as a clinical supervisor to staff within the specialist area proactively recognising the need for additional support at an early stage. Acts as a mentor to pre and post registration nurses, assisting in meeting their learning needs.

Lead on the development of local policies, protocols and procedures and monitor compliance with local and national standards in relation to specialist area of expertise. Facilitate strong clinical leadership within specialist team and ensure service needs meets the future requirements of the organisation. Research To work in conjunction with the lead Clinician to agree the priorities for the speciality research strategy on an annual basis. To provide guidance regarding clinical audit within the speciality to all members of the speciality nursing team.

To identify topics and carry out research in relation to specialist area of practice. Undertakes evidence-based practice and ensures dissemination of relevant research and best practice. Supports and advises junior staff who may wish to undertake audit and research in specialist field. To undertake membership and participation in local research and audit steering groups.

Professional Lead the team ensuring the maintenance of clinical excellence for the specialist area. Be a member of relevant professional association and attend regional/national meetings and work conferences as required. To participate in the planning, development of care priorities, strategies, and local frameworks. Identify personal learning needs and take steps to pursue personal development plan.
