Working with the Medical and Clinical Nurse Specialist Teams, the Advanced Clinical Practitioner in Clinical Haematology will be a key role in providing clinical assessment and support to patients receiving care at Leighton Hospital.

The Clinical Haematology ACP will provide the first point of contact to support clinical queries in respect of patients receiving care in:

- Inpatient wards and emergency portals
- Macmillan Cancer Care Units
- Planned Intervention Unit

The ACP will also be involved in developing and implementing care protocols.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. - To be an advanced expert who will act as an autonomous practitioner undertaking clinical assessment, diagnostics, intervention and treatment to deliver specialist care for patients with complex Clinical Haematology needs.
- To be an integral member of the Clinical Haematology Medical and CNS Team
- To review referrals from inpatient and day units, providing triage and management or onward referral as appropriate
- To provide assessment of patients in day unit (MACCU, PIU) settings to provide advice on fitness for interventions or treatments relating to management of Clinical Haematology conditions
- To support with the blood review process, including triggering and completing blood transfusion prescription
- To support with development of protocols for workload streaming and management
- Work collaboratively with the Clinical Haematology Consultant and Middle Grade medical team, including involvement in ward reviews, and workload management (including referral allocation management)
- To provide clinical advice to the Clinical Nurse Specialist Team
- To provide clinical assessment and advice to patients contacting the service will clinical queries
- To develop and participate in delivering nurse-led outpatient clinics
