An exciting opportunity has arisen for a highly motivated individual to work a long side the multidisciplinary team to assist in the development of the Alcohol Care Team within our acute care trust at Mid Cheshire Hospitals NHS Foundation Trust. The role will primarily involve the coordination of the day to day provision of support and education to this patient group ensuring their needs are met and high standards of care are delivered trust wide collaborating with members of the multidisciplinary team as required.

To be successful in this venture the post holder will need to play an active role in developing systems which support effective management and care of individuals who present to hospital with either primary or secondary issues relating to alcohol.

The Assistant Practitioner will provide support to the Hepatology Advanced Clinical Practitioners [ACP] and patients’ who have hepatological related health problems. The post holder will assist with the assessment and management of patients, they will undertake a range of administrative and practical skills including independent fibroscanning, to an expert level. The successful applicant will form a professional judgement of an individual’s health condition and identify appropriate investigations to be carried out according to the correct protocols. The post holder will manage their own role with guidance available from the ACP, supervision and teaching of staff will also be a part of this position as well as liaising with the multi-disciplinary team.

During the Coronavirus pandemic, the Trust was recognised nationally for its ‘Be Safe Be EquiPPEd’ campaign, which aimed to make the Trust as safe as possible for staff and patients, through supporting the correct use of PPE.

The results in the 2020 national NHS Staff Survey showed improvements in all elements of the safety culture theme, with 92% of respondents feeling their role made a difference to our patients. We were also recognised nationally for our workforce health and wellbeing initiatives.

Deliver clinical and therapeutic care, within the clinical setting within set protocols and within the scope of the assistant practitioners remit.

Organise and prioritise time effectively.

Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner), and deliver high standards of care in line with the plan of care.

Implement and evaluate individual care/treatment plans. Ensure care is documented accurately and in a timely manner as per Trust policy.

Support the safe admission, transfer and discharge of patients in accordance with Trust policies, under the supervision of the Specialist Nurse.

Provide high quality patient care, appropriate to patient need.

Take responsibility of care for a group of patients under the supervision of the Specialist Nurse.

Undertake nutritional assessment and ensure the appropriate nutritional intake of all patients considering any specific dietary needs and refer to the dietician appropriately.

Take personal responsibility for ensuring that patients are treated with courtesy at all times, the individual needs of the patient are respected and confidentiality is maintained at all times.

Promote and value the rights, responsibilities and diversity of people.

Work effectively as part of a multidisciplinary team recognising the contribution of all team members.

Actively participate in team development and make suggestions to the Specialist Nurse on service improvements.
