We are seeking a highly skilled Advanced Nurse Practitioner to join our team in Crewe, delivering high-quality patient care in a busy general practice.

About the Role

The successful candidate will work as part of a multi-disciplinary team, providing safe and effective care to patients. This role involves triage and treatment of patients face-to-face and via telephone, performing specialist assessments and making referrals as necessary.

As an Advanced Nurse Practitioner, you will be responsible for assessing, diagnosing, and treating patients in practice premises or their own homes. You will also refer patients to the appropriate agencies when necessary.

Key Responsibilities
  • Deliver high-quality patient care in accordance with NHS standards and guidelines
  • Work collaboratively with the multi-disciplinary team to achieve excellent patient outcomes
  • Assess, diagnose, and treat patients using your advanced nursing skills and knowledge
  • Refer patients to other healthcare professionals as necessary
Requirements

To be successful in this role, you will need:

  • MSc or equivalent relevant nursing/health degree
  • Experience of working in General Practice
  • Clinical Governance
  • Delivery of QoF targets
  • Self-audit and reflection
  • Organised and efficient record keeping and completion of paperwork
  • Time management skills, being able to prioritise work and work under pressure
  • Excellent people skills and empathy
  • Team player
Working Arrangements

This is a full-time position, working a standard 37-hour week. The salary for this role is £40,000-£60,000 per annum, depending on experience.
