About the Role

We are seeking an exceptional individual to join our Frailty team as a Trainee Advanced Clinical Practitioner. This role requires a highly motivated and experienced healthcare professional with a passion for delivering high-quality patient care.

Key Responsibilities
  • Work alongside the medical and nursing teams to promote high-quality patient-centered care.
  • Assess, plan, implement, and evaluate the immediate care and subsequent management of adult patients.
  • Provide inspirational and visible clinical leadership for the frailty team.
