Advanced Practitioner Expert

An exciting opportunity exists for a Trainee Advanced Clinical Practitioner to join the expanding Frailty team at Mid Cheshire Hospitals NHS Foundation Trust. This role offers a chance to work in a brand-new Frailty Unit and collaborate with community teams to support a virtual ward.
Main Responsibilities:
As a Trainee Advanced Clinical Practitioner, you will work alongside medical and nursing teams to deliver high-quality patient-centered care within a supported environment. Following successful completion of the MSc, you will assess, plan, implement, and evaluate immediate care and subsequent management of adult patients, liaising with middle-grade medical staff. You will utilize specialized clinical and theoretical knowledge to assess health status through physical examination, medical history, and psychosocial history, demonstrating a systematic approach to problem-solving. Once qualified, you will support the provision of specialist education and training to facilitate development among other multidisciplinary team members.
Key Areas of Responsibility:
The post holder will contribute to education and training of the multidisciplinary team, engaging in clinical supervision, self-reflection, research/audit, and quality initiatives that improve care for adults presenting to MCHFT. You will work within agreed protocols and guidelines, exercising independent judgment in history taking, clinical examination, assessment, diagnosis, and treatment of a diverse caseload of patients with emergency presentations and frailty syndromes.
