**Role Overview**
The Clinical Lead will work in partnership with the multidisciplinary team to promote high-quality patient-centered care within a supported environment.
This role requires excellent communication skills and the ability to make evidence-based clinical decisions without protocols or guidance when necessary.
The successful candidate will be responsible for assessing, planning, implementing, and evaluating the immediate care and subsequent management of adult patients.
Liaising and reporting to middle-grade medical staff, the ACP will assess the health status of patients through complete physical examination, medical history, and psychosocial history, demonstrating a systematic and thorough approach toward problem-solving.
The post holder will contribute to the education and training of the multidisciplinary team and engage in clinical supervision, self-reflection, research/audit, and quality initiatives that contribute to the improvement of care for adults presenting to MCHFT.
**Key Responsibilities**
- Provide inspirational and visible clinical leadership for the frailty team to the nursing and medical teams working within the Emergency Department, Ambulatory Care Unit, and Acute Medical Unit.
- Work alongside the medical and nursing teams to promote the delivery of high-quality patient-centered care within a supported environment.
- Assess, plan, implement, and evaluate the immediate care and subsequent management of adult patients.
**Requirements**
- Clinical based Master's Degree in Advanced Healthcare practice.
- Detailed knowledge of professional accountability and impact of NHS issues.
- Excellent communication skills in complex and challenging situations.
**Salary:** £55,000 - £65,000 per annum
