Patient Support Co-ordinator

The Home Hospice Care Team (HHCT) sits within the wider Oxfordshire Rapid Intervention for Palliative and End of Life Care project, as a separate team. The Home Hospice Care Team primarily provides care to people who are likely to die within the next 2 weeks and are at home. The team is also required to provide short term support to people who are in crisis. We are looking to recruit an Patient Support Co-Ordinator to join our team. We are seeking an individual who is proactive and committed to work as part of a team and on their own.
The postholder will play a pivotal role in the referral pathway by ensuring suitable referrals are picked up in a timely manner. The postholder will also be responsible for ensuring visits are planned in the most appropriate and efficient way.
The service operates from 7am-10pm 7 days a week, therefore the postholder is required to work 1 weekend (Saturday and Sunday), 9am-1pm a month.
The post is part time 22.5 hours per week (3 days) including 1 weekend a month.

Act as a central point for all patient referral enquiries, including monitoring Home Hospice's inbox
Create strong links with internal and external colleagues to continually improve the referral pathway into the team
Participate in daily teleconferences with colleagues and external teams.
Ensure visits are scheduled in the most appropriate and efficient way.
The Trust comprises four hospitals - the John Radcliffe Hospital, Churchill Hospital and Nuffield Orthopaedic Centre in Headington and the Horton General Hospital in Banbury.
