We are looking for an experienced and Passionate Mental Health Care Navigator to work as part of our HMP Holme House Prison & Secure Care Team.

Opened in 1992 and expanded later that decade, Holme House is a large category B prison in Stockton-on-Tees. The facility holds men from categories B, C and D as well as young adults.

How you will make a difference:
I provide a seamless package of support to ensure holistic and meaningful support is provided to those with health vulnerabilities on their release from prison, ensuring continuity of care and fruition of release plans. Reconnecting individuals with health services to reduce health inequalities and in turn reduce reoffending. The Care Navigators are embedded in integrated mental health teams and community settings, providing 1:1 and group assessment and support in a variety of settings and hold highly autonomous roles. Care Navigators, use a coaching framework, to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending. By preventing this decline in health, through robust reconnection with health services in the community we will reduce health inequalities for those leaving the criminal justice system and provide support to allow a seamless, and effective transition to independent community living.
