An exciting and new opportunity has become available within our cancer team working as a pathway navigator. The role of the Cancer Pathway Navigator is to provide effective, caring and compassionate advice and interventions to optimise patients’ timely access to cancer services, through close working with patients and multidisciplinary teams in primary and secondary care.

You will predominantly support the patients’ journey from their initial referral to first appointment and then through to diagnosis or discharge. You will be providing information, offering support, and acting as a key contact to ensure that hospital appointments and diagnostic tests are suitable and timely and that the patients are able to attend.

The role will require highlighting any potential pathway delays and working in collaboration with the relevant clinical teams and cancer services team to mitigate delays

The post holder will be required to work with the clinical teams to ensure that the patient and carer experience is central to every stage of their pathway.

Acting as the first point of contact for patients, relatives and carers
- Work with patients, families and carers to ensure they are informed, available and prepared for all appointments and investigations supporting rapid diagnosis.
- Signpost patients and their representatives from vulnerable groups to local resources that support patient access to cancer services such as transport, advocacy and buddy systems.
- Work with booking teams managing two-week wait referrals for suspected cancer to optimise patient flow and identify areas for improvement.
- In conjunction with the Cancer Nurse Specialists (CNS)/AHP’s, and Cancer Care Co-ordinators provide co-ordination of high quality patient care through on-going telephone/face to face contacts and proactive identification of needs.
- Collaborate and communicate with the wider Health Care and Cancer Services Team to ensure the provision of appropriate services to maximise the health and quality of life for people affected by cancer, improving the quality and efficiency of health care delivery and achievement of the 62 day target
- Work closely with MDT co-ordinators and cancer trackers as appropriate to ensure timely and efficient progress of patient care to prevent breaches of cancer waiting time targets.

Working with the wider MDT and cancer services teams to improve current pathways and processes resulting in better quality care and outcomes for people affected by cancer.

Acting as the first point of contact for patients, relatives and carers

Assisting the Clinical Nurse Specialists with administration duties including recording patient data

Participate in site specific and general patient experience surveys and audits

Completing competencies that are required in the site-specific team

Completing any duties delegated by the CNS within own limitations

Assist in scoping current ways of working to develop future services.

Be responsible for the regular monitoring, auditing and reporting of compliance with the pathways in line with the governance protocols.

Interpret and analysis data to produce relevant reports containing multiple pieces of information.
