Outreach Navigator – Helping Rough Sleepers Transform Their Lives

Do you want to make a meaningful difference in the lives of people experiencing homelessness? We’re looking for compassionate and driven Outreach Navigators to join a dedicated team working to support rough sleepers with complex needs. If you thrive on making an impact, have your own transport, and can work flexible shifts, including early mornings, late nights, and weekends, this role is for you.

Benefits:

* Salary: £26000-£29000pa d.o.e

* Hours of work 37hrs – Rotating Shifts

About the Job:

* Outreach Support: Engage with rough sleepers through early morning (6am-2pm) and late-night shifts (4pm-midnight), offering practical support and connecting them to essential services.

* Personalised Guidance: Provide one-to-one advocacy and support, addressing barriers such as housing, mental and physical health, substance misuse, and welfare access.

* Recovery-Focused Interventions: Help individuals build confidence, develop resilience, and work toward positive lifestyle changes in group or one-to-one settings.

* Multi-Agency Collaboration: Work with local authorities, outreach teams, and service providers to secure housing, healthcare, and support tailored to individual needs.

* Ongoing Casework: Maintain a caseload, creating personalized support plans, ensuring accurate and confidential record-keeping, and guiding clients through their journey from the street to stability.

About you:

* Experience in supporting individuals with complex needs such as homelessness, addiction, or mental health challenges.

* A compassionate, trauma-informed approach to providing care.

* Flexibility to work early mornings, late nights, weekends, and bank holidays on a rota.

* Practical Skills, including knowledge of housing options, budgeting, and accessing community services.

* Strong communication and teamwork abilities, with experience working alongside partner agencies.

* A full driving license and access to a vehicle.
