The Patient Discharge Coordinator will work as part of the Transfer of Care team with an identified care group to support the planning and delivery of actions to support the transfer of care of patients that require onward care to support discharge from hospital. The post holder will work within an agreed set of standing operating procedures and will be supported by the clinical team within the transfer of care team.

Coordination of completion of paperwork specific to support the discharge planning of patients requiring onward care following hospital admission. To advise and assist ward teams with simple and complex discharges.

Attend daily board rounds and MDT meetings to advise on discharge planning for those patients requiring onward care and ensure the medically fit status and accurate discharge plans are in place on the ewhiteboard, seeking support from clinical members of ToC team where appropriate.

Proactively encourage and/or plan all discharge activities to ensure patients only stay in hospital for the time required to complete their acute treatment and to facilitate timely and seamless discharge/transfer to another care setting.

To proactively chase and liaise with the relevant teams when the discharge is dependent in a specific action, i.e. PCR tests for patients discharge to a care home.

Raise and process relevant and timely referrals as and when required to assist discharge planning.

Act as the central point of contact for discharge planning to an agreed clinical area, provide regular discharge updates to patients, family members, ward teams, internal and external support services/agencies and escalating patients requiring input of clinical team to ToC nurses.

Flexible daytime shifts may be required, with weekend working to meet the service need.
