An exciting opportunity has arisen for a Band 5 Healthcare Transition Co-ordinator to join the Transition Team. This is a temporary 12 month post to cover maternity leave, part time hours as part of a job share will be considered.

The healthcare transition team is a small friendly central team, based in the main acute site of Sheffield Children’s Hospital. The team provide support across the trust, covering all care groups advising specialties throughout the transitional process.

This post is a non clinical role, degree level qualification or equivalent relevant experience of working in healthcare are essential.

You will be a passionate advocate for the healthcare transition of young people. This is an exciting and challenging opportunity to help facilitate the move from paediatric to adult services.

You would be supported to develop your own skills in assisting the team(s) to deliver care that makes a difference to the lives of children and young people.

- Provide non-clinical support and advice to specialties teams, young people and families during the transitional process.
- Support Pathway.
- Coordinate and support the young people in the running of the Youth Forum jointly with Communications.
- The Complex Transition Support Pathway is for teams to request additional support with transitional planning. Oversight of all young people within the pathway to co-ordinate and support teams, young people and families.
- Assist and support the Healthcare Transition Team to develop and deliver healthcare transitional pathways packages and strategies.
- Work with internal and external young person’s services to improve support networks for the young people in the transition process.
- Promote developmentally appropriate healthcare within the Trust.

- To support and work with young people and their families within transitional pathways.
- Coordinate the Complex Transition Support Pathway with support from the Healthcare Transition Nurse Specialists
- Act as an advocate for young people and families
- To effectively respond to enquiries seeking advice and support as required
- Develop and maintain up to date knowledge of trust transition guidelines and policy.
- To assist in the organisation and co-ordination of the internal and external Healthcare Transition meetings including minute taking.
- To support and work with the Healthcare Transition Team in the delivery of transition training across the trust.
- Contribute and support the Healthcare transition team with the completion of audit and service evaluation.
- Highlight barriers during transitional processes and pathways.
