We are recruiting for a Care Co-ordinator to join our expanding Sheffield office where we support Sheffield local authority, supporting the Elderly in their own homes.

You will be self motivated, reliable and keen to learn, have a good telephone manner with the ability to work under pressure whilst adhering to company policies and procedures. Previous Care Coordination experience would be ideal but training can be given but you MUST have experience of working in the social care sector.

Full driving license with use of own transport is a must

Responsibilities include
- Ensure that the Quality of the Service meets with all policies and expectations.
- Ensure that all Regulatory requirements are met.
- To implement a high quality service through the effective matching of service user needs with suitably qualified and experienced care workers
- To facilitate effective communication between the Organisation, service users, care workers and outside Health Professionals and Local Authorities.
- To ensure a high level of records are held for both Care Workers and Service Users, updating on a regular basis due to changes and on completion of annual reviews
- To ensure all aspects of health and safety requirements are adhered to as specified within the Health and Safety Policy and Procedure and in accordance with training given.
- At all times observe and respect the service user’s dignity, privacy and independence as far as is practical.
- Building of staff rota for the Carers using the careplanner system.
- Ordering of client medication on a weekly basis, check all medication received; deliver to clients in a timely manner.
- Any other duties as advised by your line manager that are deemed as necessary for your role
- At times of holiday or sickness or in the event of that the company is short of staff you will provide cover for visits and this could include late night and weekend.
- Participate in the out of hours emergency on call duties with other team members.

In return we are offering a competitive salary up to £24k.

Bi annual bonus based on KPI's

Further training to Level 5 Health and Social Care Management

**Salary**: £22,000.00-£24,000.00 per year

Schedule:

- Monday to Friday
- Overtime
- Weekend availability

Licence/Certification:

- Driving Licence (required)
