An exciting opportunity has arisen for a Band 5 Youth Liaison and Engagement Co-ordinator to join the Healthcare Transition Team.

This is a 1 year full time post with a view to extending; part time hours as part of a job share will be considered.

The healthcare transition team is based in the acute hospital site of Sheffield Children’s. The team provide support across the Trust, covering all care groups advising specialties throughout the transitional process, as well as supporting and co-ordinating our young people in the running of the trust youth forum.

We are looking for someone with lived experience to join our small welcoming team.

A really important element of this nonclinical role is building a trusting relationship with young people, demonstrating empathy and knowledge of what young people may be going through.

You will need to be able to connect with young people, speaking to them on their level. Supporting and advocating for young people to make a difference in their lives as well as promoting their voice and ensuring this is heard in their health care journey.

Degree level qualification or equivalent relevant experience are essential.

You will use your own lived experience to work with young people in our care aged between age 12 and 16 (18 in some cases).

You will support and advocate for young people to help them express what they want for their care.

You will bring a unique perspective, having navigated similar challenges, which enables you to connect empathetically with patients.

You will work with youth forums and transition teams to make sure young people's voices are heard. This includes sharing feedback about what young people think so that the Trust can develop services.

Sheffield Children’s is one of three independent, specialist paediatric hospitals in the country, providing dedicated healthcare for children and young people across community, mental health and acute specialist settings.

Provide empathetic support to young patients, helping them navigate their healthcare journey by drawing upon your own experiences.

Advocate for the needs and preferences of young patients, helping them articulate their concerns and expectations regarding their medical care.

Initiate contact with patients under the Trust's care to understand their experiences. Provide support and advocacy for individuals with long-term health conditions and those transitioning into adult care.

Be a communication bridge between young patients and healthcare professionals, ensuring their voices are heard in treatment planning processes.

Engage in hospital activities, including youth forums and transition teams, contributing to discussions that shape the hospital experience for young patients.

Establish a structured feedback mechanism to gather insights from young patients about their hospital experiences, enabling continuous improvement.

Foster positive collaborations with clinicians, offering insights that contribute to a patient-centric approach in the hospital's care delivery.

Analyse trends in patient feedback to identify common themes, providing valuable information for adapting hospital services to better meet the needs of young patients.
