In our quest to deliver patient-focused quality care we are looking to recruit a highly motivated, innovative and enthusiastic Mental Health Transfer Clinical Lead to join our forward thinking, friendly and expanding Health & Justice team based at HMP/YOI Bronzefield. (This post includes travel to sites within the Surrey cluster.)

The p/h will support the Healthcare teams by having day to day responsibility & oversight for the MH transfers across the 5 Surrey prisons; HMPs Bronzefield,High Down,Downview,Send& Coldingley.
The p/h will have clinical oversight of all MH transfers across Surrey ensuring effective communication between the Health & Wellbeing Teams, the prison & community providers as well as our NHSE Commissioners.
The p/h will maintain effective & efficient resource utilisation, the delivery of evidence-based patient care & a safe service. In conjunction with Consultant Psychiatrists & Service Managers, the post holder will provide clear, visible leadership in the planning & implementation of managing transfers across the prisons & support the teams in creating effective patient care pathways.
The p/h will ensure the timely assessment & transfer of acutely mentally ill patients under the Mental Health Act 2007. Liaising with the courts, L&D services for those that are bought into custody under paragraph 5 as a place of safety. Involving CNWL senior management & NHSE commissioners as required
The p/h will be expected to operate flexibly within a challenging healthcare environment, involving multiple providers, whilst being an ambassador for the HWB Teams & CNWL.
The p/h will act as a resource within CNWL concerning service development, taking forward initiatives to share best practice & acting as a clinical role model within the team by demonstrating high levels of clinical practice.

**Mental Health Act Transfer Coordination**:
E nsure timely assessment and transfer of acutely mentally ill patients under the Mental Health Act 2007.
Support Consultant Psychiatrists and other colleagues in relation to all patients identified for possible transfer under the Act
Liaise with Psychiatrists and others carrying out assessments for transfer, ensuring appointments are booked in a timely fashion and carried out to completion
Continually liaise with mental health inpatient units in relation to those identified for transfer - in particularly with bed managers - to ensure any delay in transfer is prevented or minimised and individuals are transferred as soon as
