Internal Discharge Lead - Band 8A
This role is a new exciting opportunity to make a difference to patients who are medically fit and are ready to return home.
We are looking to welcome a Lead Nurse or Allied Health Professional to our Internal Discharge Team who has a passion to put the person at the centre of what we do. If you’re someone who works to empower the needs and the voice of the people, we provide care for and who recognises the need to support people’s care when they are discharged home - this could be the perfect role for you.
About the Internal Discharge Service
The service is part of the Corporate Division and is integral to hospital flow. This is a 7-day service and currently operates between 0800 - 1700hrs.
You will provide professional leadership and guidance to the team in ensuring those that we provide care for are safely supported home.
You will join our service as we look to expand the discharge options across our system and work to provide seamless care across the four counties that access the Royal United Hospital.
Work alongside colleagues from the Bath, Swindon and Wiltshire CCG. You will work closely with colleagues who provide in-reach clinical reviews to support efficient and high-quality discharge planning. Including our inpatient services and reducing length of hospital stays and ultimately ensuring positive outcomes for the people we provide care for.
