As pressure on emergency departments continues to grow, new care models and ways of working are pivotal in relieving pressure through admission avoidance and facilitating safe early discharge.

We are offering an exciting opportunity for highly motivated, dynamic, and inspirational individual to join our team to help lead and shape our care models working with similar minded clinicians about the system.

In this role you will be supported by a team of senior clinicians to provide ongoing personal and professional development.

Contribute to the design/delivery of urgent primary care and intermediate care services, aiming to provide safe and appropriate careat home aligned to the Health Board’s Urgent and Emergency Care programme and its agreed principles/ scope

Work in partnership with the Urgent and Intermediate Care and WAST multidisciplinary team to provide remote clinical triage and assessment to patients who have presented to WAST and whose conveyance to hospital could be avoided

Arrange referrals to community pathways (eg; mental health services, intermediate care crisis response, patient’s own GP), including scheduling of appointments in emergency units and SDEC

Work with key multi agency team members to support the development of a coordinated clinical streaming hub

Develop and strengthen interprofessional working between key stakeholders in Urgent primary/ emergency care to support the development of a place-based county-wide clinical response team

Support opportunities for innovation and implementation including the development of a community diagnostics strategy, providing care closer to home

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
