Critical Care Assistant Practitioner

The Assistant Practitioner role is designed to deliver evidenced based clinical care under the direction and support of a Registered Nurse by following the Health Board’s protocols, policies, procedures pathways and care plans.
The post-holder will;
- be responsible for the delivery of a high standard of safe, individualised, efficient, cost-effective care under the delegated authority of a Registered Nurse.
- communicate effectively at all times and in conjunction with other ward/team members, contribute to and maintain the unit/team philosophy.
- practice in accordance with Health Board standards and statutory requirements and operate within the boundaries of the role and assessed competencies.
- develop the role as required within the boundaries of the KSF post outline and agreed competencies.
- work a Monday to Friday roster, which will not include nights, weekends or bank holidays.
The main duties of this role are outlined below.
Undertake identified allocated duties in relation to the care environment, the direct care of the patient/client as identified by a Registered Nurse and in accordance with Hywel Dda Health Board policies and procedures.
Perform duties and activities within the framework of a plan of care, as set and reviewed by a Registered Nurse at regular intervals.
Assist patients/clients to achieve physical and emotional well-being and comfort. Ensure that any changes in patients/client’s condition or circumstance are reported immediately to the appropriate person/Registered Nurse/nurse in charge. Utilise transferable skills within level of competence.
The Critical Care Associate Practitioner (CCAP) will receive training and education to undertake additional duties and will complete the in-house advanced training programme for the Critical Care Associate Practitioner.
The CCAP will provide essential nursing care for patients categorised as requiring Level 1, 2 and 3 care - with an identified qualified nurse as a supervisor. The training will allow the CCAP to assess, plan, implement, and evaluate the care they’ve provided to the allocated patient alongside their supervisor (Registered Nurse), who ensures that support and advice is available for any aspect of care that is not within the realms of the post holder’s capabilities / responsibilities.

**Four main hospitals**: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
**Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales
