**Role and Responsibilities**:
As a Care Coordinator, you will play a crucial role in ensuring the smooth and effective delivery of care services to our clients. Your responsibilities will include:

- **Client Assessment**: Alongside the quality assurance team you will be taking part in assessments of clients' care needs and creating care plans.
- **Scheduling**: Coordinating and scheduling care visits, ensuring appropriate allocation of care staff based on clients' needs and preferences.
- **Communication**: Maintaining regular communication with clients, their families, and care staff to monitor progress and address any concerns.
- **Staff Management**: Supervising and supporting a team of care assistants, providing guidance, training, and feedback.
- **Quality Assurance**: Monitoring the quality of care provided and implementing improvements as needed to uphold our standards.
- **Documentation**: Keeping accurate and up-to-date records of client care plans, visits, and any changes in their conditions.
- **Collaboration**: Collaborating with healthcare professionals, social workers, and other relevant parties to ensure holistic care.
- **On Call**:alongside the already established team, you will take part in the on call rota - this will include working evenings and weekends.

**Qualifications and Requirements**:

- Previous experience in a care coordination role or a similar position within the domiciliary care sector is preferred.
- Strong organisational skills and the ability to multitask effectively.
- Excellent communication skills, both written and verbal.
- Compassionate and empathetic nature, with a genuine interest in the well-being of others.
- Proficiency in using computer software for scheduling and documentation purposes.
- Knowledge of relevant regulations and guidelines in the domiciliary care sector.
- NVQ Level 3 in Health and Social Care or equivalent qualification is desirable.

**This position is a temporary role to cover a period of maternity leave**.
